Description & Directions for Filing a Claim

If you received a damaged, missing, defective, or incorrect product, please complete this form to submit a claim for review by our team. To ensure your claim can be processed efficiently, please provide complete and accurate information regarding your order, including invoice details, product information, and supporting photos. The information submitted through this form will allow our team to identify the shipment, review the issue, and determine the appropriate resolution. Claims should be submitted within 48 hours of delivery whenever possible, especially for shipments that may have been damaged during transportation. Submitting claims promptly allows us to work with shipping carriers to investigate and resolve the issue more effectively. Before completing this form, please gather the following information and documentation:

• Invoice number, purchase order number, or order number
• Product SKU and product name
• Quantity affected
• Date the shipment was delivered
• Name of the shipping carrier (UPS, FedEx, freight carrier, etc.)

In addition to the order information, clear photos are required to help us properly evaluate the claim. Please upload images that show the condition of the shipment and the product.

Required photos include:

• The outer packaging or shipping box
• The shipping label attached to the package
• A close-up photo showing the damaged or defective area
• A full photo of the entire product

Additional photos or documentation may also be uploaded if available. Providing detailed descriptions and clear images helps our team review claims quickly and coordinate with shipping carriers when necessary. Once your claim is submitted, our team will review the information and may contact you if additional documentation or clarification is required. Most claim reviews are completed within 7–10 business days. After the review is finished, we will contact you regarding the status of your claim and the next steps. Possible resolutions may include:

• Replacement product
• Credit to your account
• Further instructions for resolving the issue
